Park River Estates Care Center for Your Transitional Care
- Experienced Caregivers -
We know that recovering from surgery, injury or an illness can be challenging. Our therapy team, professional nursing staff, and clinical team work together to make your recovery easier.
Park River Estates works together with Centrex Rehab, a team of physical, occupational, and speech therapy professionals who specialize in geriatric and rehabilitative care.
- Support with Your Transition -
We make your transition process simple. Discharge coordination and planning begins immediately. Our team collaborates to put together a person-centered care plan, helping you to regain strength and reach your goals.
During your stay, we compile all the necessary resources and tools needed to ensure you are prepared and confident when you return home.
Before discharging, you will be given a personalized take home care plan with instructions, future appointments, resources and contacts to keep you progressing.
After returning home, Park River Estates continues to be a resource and support system for you. Our Licensed Social Workers will follow up with you 48 hours after your discharge home and again in 7 and 21 days.
- Recover in Comfort -
Park River Estates provides our guests with a safe,
comfortable, private and home-like atmosphere.
We offer 14 private Transitional Care Suites. Each suite includes a 32' flat screen television, large reclining chair, cable, wireless internet, large private bathroom, armoire, locked safe and nurse call light system.
Guests and families are also welcome to relax in our Outdoor Courtyard, Veteran's Memorial Library, Patient & Family Lounges and our Chapel.
- Accommodations -
We offer both family style dining in our Transitional Care Dining Room or private in-suite delivery meal service. We provide multiple menu options
designed to meet each patient's nutritional needs and preferences.
Housekeeping and on-site laundry services are provided daily. Our hair salon offers full salon services by a Licensed Stylist.
-Therapeutic Programs-
In between therapy sessions, our guests have full access to participate in scheduled Therapeutic Recreation activities, our Pet Therapy Program from licensed therapy dogs and leisure activities.
Our Chaplain provides individual spiritual support for all guests and their families. Our Chapel holds Catholic Communion and Spiritual Services weekly from our community churches.
